ELIGIBLE SALE CRITERIA:
In order for a sale to be considered eligible it must meet
the following criteria:

Agent/sales person must be registered at hsa.cordestech.com
by December 31, 2005 at 1 p.m.

HSA accounts must be completely set up and fees paid
between October 15, 2005 and January 15, 2006.

Must be a new account at BASIC or transferred from another
bank (renewals at BASIC are not eligible)

HSA must be in place on January 15, 2006.
-
When a group or individual enrolls they must list agent/sales
person on appropriate paperwork in order for agent/sales
person to have a credited sale.
