The first step in setting up the BASIC HSA
is for you to contact BASIC by phone or e-mail. Our toll-free
number, (800) 444-1922 ext.3, is available from 9:00am- 5:00pm
EST; we can also be contacted at sales@basiconline.com .
The next step is to consult your BASIC sales representative
or Insurance Agent to develop a plan design that is right for
your company. Once that is determined, it is time to choose
an enrollment method; on-line via the Sign Up link or
download and print all forms. Complete and fax or mail the "Employer
Information", the "Bank Account Funding Form"
and the "BASIC HSA Administration Agreement" to BASIC
at Fax: 1-866-HSA-7572 or BASIC HSA, 9246 Portage Industrial
Drive, Portage, MI 49024.
Once all employees complete the enrollment forms please mail
or fax to BASIC. You also need to e-mail the contribution spreadsheet
to HSA@basiconline.com . Once this is completed, the HSAs are
set up and administrative fees and contributions will be collected
through two separate ACH transactions from the provided checking
account. Your employees will receive their debit cards in seven
to ten business days. At this point, monies will be deducted
from your employees' paychecks through your payroll system and
put into their HSAs. Your employees can now use their debit
card for qualifying medical and dental expenses.
