We have created this online process to give you more flexibility
to present the BASIC HSA™. BASIC remains committed to
working closely with you and your groups. We would be pleased
to talk and/or meet with you and/or your group to explore and
implement the BASIC HSA™ or any of our other services.

Steps to getting a proposal:

Enter the requested information on the HSA Information
Form below.
a. This information will be used to generate the proposal,
the better the information the better the proposal.
b. There are NO required fields when entering data…the
more you enter the better the proposal information.
c. HELP with the field definitions is available by clicking
on the ? mark next to the field’s title.

Click the “Submit” button.
a. You will be promted to open a Microsoft Word (rtf) file.
Click OPEN when prompted to view the proposal online.
b. You will receive by email a Microsoft Word document
that contains a proposal.
c. BASIC will also receive a copy of the proposal to better
assist you if you have questions.
