Follow the steps below when you are setting up an individual with the BASIC HSA™.

1. Have individual go under “HSA Forms”
2. They can then download and print all forms under “Individual Enrollment Forms”
3. Individuals should complete the forms and fax or mail them to BASIC

4. BASIC will set-up the individual HSA upon the receipt of all forms
   Individual will receive their debit card 7-10 days after set-up
